For over a century, hypothyroid care has relied primarily on levothyroxine (T4). Yet modern studies confirm what many patients report: laboratory “normal” doesn’t always mean feeling well.
In recent research, up to twenty-five percent of patients on T4 alone continue to experience persistent symptoms—such as fatigue, mood changes, and cognitive fog—even after their thyroid-stimulating hormone (TSH) is normalized.
New randomized trials show that adding the active thyroid hormone T3 to standard therapy—either as synthetic liothyronine or as part of natural desiccated thyroid extract (DTE)—can help correct low T3/T4 ratios and, in some cases, restore energy or cognitive clarity when T4-only therapy is not enough.
The difference may be most noticeable in patients with ongoing symptoms, underscoring the need for individualized hormone replacement rather than a one-size-fits-all approach. DTE provides both T3 and T4 in a natural balance and remains a relevant option for many.
Research shows that a subset of patients prefer DTE or notice modest improvements in quality of life compared with T4 alone.
The clinical consensus is clear: For many people, tailored thyroid therapy— including combination approaches or DTE—can make a meaningful difference. If you aren’t thriving on standard treatment, talk to your doctor about all your options.
DTE Access: How to Take Action
The Alliance for Pharmacy Compounding (APC) is calling for public support to help protect patient access to DTE. The FDA’s recent reclassification of DTE as a biologic could hand market control to one manufacturer, reduce access, and increase costs.
